Durability of HBeAg Seroconversion After Adefovir Treatment

HBe seroconversion, or loss of HBe antigen (HBeAg) and development of anti-HBe antibodies, is a marker for successful treatment of chronic hepatitis B.

A study presented at the recent Digestive Disease Week 2006 conference in Los Angeles showed that 91% of patients treated with adefovir (Hepsera) had durable HBe seroconversion lasting an average of three years

This study enrolled 45 chronic hepatitis B patients with compensated liver disease who experienced HBe seroconversion and suppression of HBV DNA to below 100,000 copies/mL during treatment with 10 mg once daily adefovir. Median duration of adefovir therapy was 54.4 weeks before seroconversion and 37 weeks after seroconversion. After completion of treatment, subjects were followed for a median of 143 weeks.

Results

41 patients (91%) maintained durable HBe seroconversion during follow-up (37% followed for 2-3 years and 46% for more than three years).
Four patients (9%) did not maintain seroconversion; seroreversion (becoming HBeAg positive again) occurred at week 12 for three patients and at week 16 for one patient.
At the time of seroreversion, all four patients had HBV DNA greater than 100,000 copies/mL and ALT levels of 84-222 IU/L.
Durable HBe seroconversion, or lack thereof, was not associated with patient age, gender, baseline HBV DNA, or baseline ALT; all four patients who experienced seroreversion were Asian and had HBV genotype C.
For all patients, the median HBV DNA at the last measurement after stopping adefovir was 3.04 log copies/mL and the median ALT level was 28 IU/mL.
Median duration of adefovir therapy before seroconversion was longer among patients who eventually experienced seroreversion compared with those who maintained seroconversion (108 vs 48 weeks, respectively).
Median duration of adefovir therapy after seroreversion was shorter among patients who eventually experienced seroreversion compared with those who maintained seroconversion (22 vs 41 weeks, respectively).

Conclusion

The researchers concluded that HBe seroconversion was durable after adefovir treatment in 91% of patients over a median follow-up period of three years.
