Early HCV Viral Kinetics Predict Response to Re-treatment with Pegylated Interferon plus Ribavirin

It is well known that early response to interferon-based therapy for chronic hepatitis C predicts sustained response among patients treated for the first time, but less is known about viral kinetics in prior non-responders and relapsers.

As reported in the April 2007 Journal of Hepatology, French researchers evaluated the efficacy of 1.5mug/kg/wk pegylated interferon alpha-2b (PegIntron) plus 1000-1200mg/day ribavirin in 154 chronic hepatitis C patients treated outside of clinical trials who were relapsers (53) or non-responders (101) to previous interferon/ribavirin combination therapy; treatment lasted for 48 weeks.

Results

44 patients (28.6%) achieved sustained virological response (SVR).

Prior relapsers had a significantly higher SVR rate (58.5%) than prior non-responders (13%).

Rapid (week 4) and early (week 12) virological response had high negative predictive values for SVR (94% and 97%, respectively).

However, positive predictive values were relatively low (52% and 49%, respectively).

In a multivariate analysis, predictors of SVR were:

- relapse after prior interferon plus ribavirin (vs non-response);
- non-1 genotype;
- mild or moderate fibrosis
- baseline HCV viral load < 2 million copies/mL.

Conclusion

In conclusion, the authors wrote, "Relapsers to interferon plus ribavirin therapy, and non-responders with genotype non-1 and mild or moderate fibrosis, achieved a relatively high SVR rate following retreatment with peginterferon plus ribavirin. Early viral kinetics had a high negative predictive value of SVR."
