Hyperlactatemia: Incidence in Patients Receiving NRTIs

Case series of adults receiving NRTI with _lactate and GI symptoms and/or unexplained _ALT
Relative risk for various ART regimens compared to ZDV/3TC was estimated
33 patients met case definition during study period (7/98 and 5/00)
Incidence rate among 1611 patients for any NRTI regimen was 14.5 (95% CI: 10.0-20.3) cases/1000 person-years

• Lonergan and colleagues have been studying symptomatic hyperlactatemia in a case series of adults receiving NRTIs.1 They are defined as having an increased lactate level associated with gastrointestinal symptoms and/or unexplained ALT levels.

• After identifying these cases, the researchers looked at their therapy as well as the relative risk of developing symptomatic hyperlactatemia compared to patients who were on zidovudine and 3TC.

• A total of 33 out of 1,611 patients met the case definition between 1998 and 2000.

• The overall incidence rate for any NRTI regimen was 14.5 cases per 1000 person-years.
