EACS 2015: With EACS Release, All International HIV Treatment Guidelines Agree on When to Start

The new European AIDS Clinical Society (EACS) Guidelines, released last week at the15th European AIDS Conference in Barcelona, bring Europe into line with the rest of the world by recommending HIV treatment upon diagnosis for all patients. This is the first time since 2006 that all international guidelines have agreed on their "when to start" recommendations.

The World Health Organization (WHO) has released updated guidelines calling for universal antiretroviral therapy for everyone diagnosed with HIV, regardless of CD4 T-cell count, and pre-exposure prophylaxis (PrEP) for people at substantial risk of infection. The organization estimates that the recommendations, if widely adopted, could avert 21 million deaths and prevent 28 million new infections worldwide by 2030.

The World Health Organization (WHO) will issue new HIV treatment guidelines later this year recommending treatment for all, regardless of CD4 cell count, Dr. Meg Doherty of the WHO Department of HIV/AIDS told a satellite meeting ahead of the 8th International AIDS Society Conference (IAS 2015) in Vancouver on Sunday.

People who start antiretroviral therapy (ART) immediately after an HIV diagnosis -- while their CD4 T-cell count is still high -- rather than waiting until it falls below 350 cells/mm3 have a significantly lower risk of illness and death, according to long-awaited findings from the START trial presented at the 8th International AIDS Society Conference this week in Vancouver and published simultaneously in the July 20 advance edition of the New England Journal of Medicine.

Everyone with HIV who is prepared to take antiretroviral treatment should receive it, regardless of CD4 cell count, new draft British HIV Association (BHIVA) treatment guidelines recommend. The new draft guidelines, published for consultation this week, say that anyone living with HIV who understands the commitment of antiretroviral therapy and is ready to start should receive treatment. The change -- from a recommendation to start treatment before the CD4 cell count falls below 350 cells/mm3 to treatment for all -- follows the results of the START trial, a keenly awaited international study of when to start treatment.
